| Özet |
| A 60-year-old male with prior history of coronary artery bypass grafting presented with non-ST elevation myocardial infarction. Because both the native circumflex artery and aorta-to-circumflex bypass graft were occluded (Fig. 1a)(video 1, Supplementary Digital Content 1, http://links. lww. com/MCA/A522), an intervention was planned for the chronic total occlusion (CTO) at circumflex artery via a retrograde approach. A homemade shortened 7 Fr extra back-up (EBU) guiding catheter [1] was placed into the left main coronary artery ostium. The retrograde approach was preferred due to the ambiguousness of the proximal cap, lengthiness of the lesson (estimated over 30 mm) and well-developed collateral from the left anterior descending artery (LAD).
