IS RE-TUR B NECESSARY FOR SUPERFICIAL BLADDER CANCER?

Introduction: In this study, we aimed to evaluate the rate of residual tumors and/or recurrent tumors and based on those rates to determine whether the treatment options would change or not in patients with superficial bladder tumors who underwent a reTUR (transurethral resection) 4-6 weeks after the initial TUR. Materials and Methods: This research involves 80 patients who underwent a TUR due to a bladder tumor, and whose pathology results revealed grade 2-3 pT1 transitional cancer or grade 2-3 pTa with tumors 3 cm in size or over. None of the patients had any post-resection tumors left. 4-6 weeks later, all the patients underwent a reTUR. New tumor formations were resected wherever seen. Additionally, previous resection surface was resected. The material obtained was sent to the pathology lab in separate tubes. In case of absence of new tumor, the previously resected area was resected only. Following the reTUR, both groups of patients with and without the new tumor formations were administered intracaviter BCG (Bacillus Calmette Guerin) once a week. Results: In 59 out of 80 (73.75%) patients no new tumors were spotted following the reTUR; whereas in 21 patients new tumors were established. Out of these 21 patients, 6 of them (7.5%) had residual tumors (including 1 patient with an up grade), and 11 (13.75%) had recurrent tumors, and 4 (5%) had both residual and recurrent tumors (including 1 patient with an up stage). Compared to the initial tumor pathology, progression was seen in 2 of the patients with tumors after the reTUR. Conclusion: In high risk bladder tumors, any residual tumors can cause early recurrence and progression, so we believe the residual tumors can be removed and treatment attained through reTUR.
