Surgical repairment of traumatic rib fractures and the role of video-assisted thoracic surgery

BACKGROUND: The treatment of traumatic rib fractures depends on the number of broken ribs, the type of fracture, whether it causes chest deformity, and accompanying pulmonary pathologies. The aim of this study was to evaluate the early postoperative results of traumatic rib fracture cases for whom surgical repair was decided for certain indications in our clinic. METHODS: The data of 56 patients treated for traumatic rib fracture in our clinic between February 2022 and October 2022 were reviewed. Patients who underwent surgical rib repair were included in the study. The characteristics of the patients, the number of broken ribs, fracture type, cause of trauma, accompanying pathologies, surgical technique and postoperative follow-up findings were recorded. RESULTS: During the study period, there were 56 patients who were treated with the diagnosis of traumatic rib fracture in our clinic. A total of 11 patients who underwent surgical rib repairment were included in the study. The mean age was 40.6±13.1, the median age was 39 years (range: 21-67). The most common cause of trauma was a motor vehicle crash (54.5%). Eight of the patients had a pleural complication accompanying rib fracture. Exploration with VATS revealed coagulum deposition in the pleural cavity in 4 patients, which the thorax tube could not drain. It was not seen any intraoperative complication; however lobar atelectasis was observed as a postoperative complication in 2 patients. CONCLUSIONS: VATS-guided surgical repair of rib fractures can be preferred in the appropriate patient group with its low complication rate and effectiveness.
